CNR Health Care Network is a voluntary, not-for-profit health care organization. Our health care professionals collaborate with other community health service providers to deliver a full range of continuing care services to the elderly, frail, and chronically ill in Brooklyn and Queens. These include adult day health care, long term home health care, AIDS home care, rehabilitation, nursing home and hospice care.
