CNR's Adult
Day Health Care program is ideal if you are frail or have a chronic
condition that requires skilled nursing and rehabilitation.
At three locations, health care services and social interaction is provided
by a team of nurses, social workers, recreational therapists, nutritionists
and other health care professionals.
Participants enjoy a healthy meal in addition to services such as blood
pressure monitoring, injections and dressing changes. Parties and trips
are part of the program along with daily games, crafts and exercise.
For people covered by Medicaid, all costs are paid. If you believe that
you may be eligible for Medicaid, but are not currently enrolled, we
will help you determine your eligibility and get you enrolled.
The program is open 7 days a week, 9 a.m. to 4 p.m., except for scheduled
holidays.
